Every fall, the same question comes up constantly at our Main Street office: is this just a cold, or is it something that needs more attention? With flu, COVID-19, and the common cold all circulating in western Wayne County from October through March, and symptoms that overlap heavily in the first day or two, it's a fair question — and the honest answer is that you often can't tell from symptoms alone.

A cold usually builds gradually over a day or two, centers on the nose and throat — congestion, a scratchy throat, mild cough — and rarely comes with a real fever. The flu tends to hit fast and hard: a sudden fever, body aches, deep fatigue, and a cough that can knock you flat for several days. COVID-19 can look like either one, which is exactly the problem; the same virus can cause anything from a scratchy throat and nothing else to a flu-like illness with fever and body aches, and loss of taste or smell — once considered a telltale COVID sign — is actually less common with the variants circulating now than it was in 2020.

Because the symptoms overlap so much, testing is really the only reliable way to know which one you have, and it matters for more than curiosity. Flu has antiviral treatments, like oseltamivir, that work best when started within the first couple of days of symptoms. COVID has its own antiviral options for people at higher risk. A few signs should move you from "wait and see" to "come in today": a fever over 102°F that isn't coming down, trouble breathing or chest pain, symptoms that improve and then suddenly get worse, or a cough and fatigue that haven't budged after a week. For infants, older adults, and anyone with asthma, COPD, diabetes, or a weakened immune system, it's worth calling us earlier rather than later, since those groups are more likely to develop complications from any of the three.
